Key Responsibilities\n

• Psychosocial Assessment: Conduct thorough psychosocial assessments in a timely manner, considering the impact of social, emotional, and behavioral factors on treatment outcomes. Your assessments will guide the development of tailored intervention strategies.

• Clinical Interventions: Provide supportive counseling to patients and their families, facilitating discussions on complex decisions such as hospice care and Do Not Resuscitate (DNR) orders. Collaborate with case managers on discharge planning and develop programs that address the unique needs of the cancer population.

• Triage and Case Management: Demonstrate proficiency in managing clinical caseloads, effectively identifying high-risk situations and balancing referrals with ongoing patient care. You will work collaboratively with colleagues to seek guidance when necessary.

• Adherence to Legal and Ethical Standards: Ensure compliance with all regulatory requirements related to suspected abuse, patient rights, and ethical guidelines in social work practice. Your role will include documenting interventions and participating in case discussions to uphold these standards.

• Documentation: Accurately complete documentation as per organizational policy, which includes Interdisciplinary Plans of Care, social services progress notes, and follow-up forms related to advance health care directives.

• Staff Training: Conduct in-service training for staff members, addressing psychosocial issues relevant to patient care to enhance their understanding and ability to support patients effectively.
\n
• Quality Improvement: Actively participate in quality improvement initiatives, analyzing process and outcome indicators to refine patient care practices and enhance overall service delivery.

• Additional Responsibilities: Engage in social work activities such as organizing awareness events during Social Work Month and facilitating educational support groups for patients and families.

This position primarily focuses on patients in a school setting who may be dealing with acute and chronic health issues, with an emphasis on those impacted by cancer. You will work closely with patients and families, addressing their emotional, psychological, and social needs through a holistic lens. Qualifications\n

• A Master's Degree in Social Work from an accredited graduate school.

Preferred Experience\n

Compensation and Benefits\n
