Work Environment

This Physical Therapist position is based onsite, primarily focusing on home care settings in the Bonita Springs area. You will work directly in patients' homes, providing a comfortable and familiar atmosphere for patients to receive therapy. Additionally, you will collaborate closely with other healthcare professionals to ensure a holistic approach to patient wellness.

Key Responsibilities

• Conduct comprehensive evaluations to assess patients' functional limitations and disabilities, using tailored assessment tools to develop individualized treatment plans.

• Implement effective treatment protocols based on evaluation findings, ensuring that each patient's unique needs are met.

• Consult and collaborate with a multidisciplinary team, ensuring all modifications to the treatment plan are documented and communicated effectively.

• Maintain accurate and timely documentation of all patient interactions, including detailed encounter notes and progress summaries that reflect the patients' journeys.

• Perform ongoing assessments and modify treatment approaches as needed based on patient progress and responses to care.

• Participate in team meetings, in-services, and collaborative initiatives to enhance care delivery and improve access to services.

• Stay updated with the latest state practice laws, ensuring compliance with healthcare regulations and standards.

• Manage patient discharge planning, providing follow-up resources and support to promote continued recovery outside of therapy sessions.

Patient Population / Client Focus

As a Physical Therapist at Retain Wellness & Physical Therapy, you will work with a diverse patient population, including individuals recovering from surgeries, injuries, or chronic conditions that require rehabilitation. You will utilize your expertise to help them regain mobility, strength, and independence, improving their overall quality of life. Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program.

• A valid state license to practice physical therapy in Florida.

• A commitment to ongoing education and professional development.

• Current CPR certification.

Preferred Experience
