Position: Home Health Physical Therapist Employment Type: Full-time/Part-time Coverage Area: Northen Virginia; Culpepper, Marshall, The Plains, Hume, Linden, Manassas, Woodbridge, Gainsville Responsibilities Conduct comprehensive evaluations of patients' physical and functional abilities Develop personalized treatment plans to enhance mobility, relieve pain, and improve overall physical well‑being Provide hands‑on therapy and exercises to patients in their home settings Educate patients and their families on therapeutic exercises and techniques Collaborate with interdisciplinary healthcare professionals to coordinate patient care Document and maintain accurate records of patient progress and treatment outcomes Qualifications Valid license as a Physical Therapist in Virginia.
