Key Responsibilities\n

• Conduct thorough assessments of patients' psychosocial needs and establish appropriate treatment plans that may include case management, short-term counseling, and crisis intervention.

• Assist patients in navigating the complex healthcare system by helping them fill out forms for entitlements, supplemental insurance, and benefit programs, aimed at minimizing their reliance on emergency assistance.

• Coordinate with pharmaceutical companies and community resources to help patients access necessary prescription cost assistance programs.

• Act as a patient advocate, interfacing with community agencies and resources to facilitate access to services and support networks.

• Maintain meticulous and up-to-date patient records using our electronic health record (EHR) systems, ensuring that all documentation meets regulatory standards and is completed in a timely manner.

• Arrange transportation assistance for patients when necessary, coordinating logistics to help them attend appointments and access necessary services.

• Participate in clinical meetings and case conferences to actively collaborate with a team of healthcare professionals, sharing insights and strategies that enhance patient care.

Qualifications\n

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or related counseling field is required.

• Possession of an active or independent license in one of the following: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

Compensation and Benefits\n
