Key Responsibilities\n

• Conduct thorough evaluations and assessments to understand the needs and circumstances of patients and their families.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP) tailored to each patient.

• Monitor and analyze outcomes, contributing insights that help refine services and interventions.

• Develop trusting relationships with patients, families, and colleagues to foster a positive and effective caregiving environment.

• Provide essential support and resources to families, empowering them to navigate the complexities of end-of-life care.

• Ensure compliance with federal and state regulations and organizational standards to maintain high-quality service delivery.

• Utilize evidence-based practices to formulate strategies for patient support and family engagement.

• Engage in ongoing professional development to remain informed about the latest methodologies in social work and patient care.

Patient Population / Client Focus\n

The ideal candidate will work closely with individuals and families at the hospice center, many of whom are confronting terminal illnesses. The social worker’s role involves understanding the emotional and psychological challenges these families face, as well as providing appropriate interventions and support tailored to each unique situation.

Qualifications\n

• Must possess a Master's Degree or a Doctorate in Social Work.

• Active LCSW licensure issued by the Department of Public Health.
\n
• A valid driver’s license is required to facilitate potential home visits.
\n
• Strong commitment to implementing Natural Learning Environment Practices, enhancing the quality of care.

• Exceptional organizational skills and the ability to manage multiple cases effectively.
\n
• Ability to demonstrate empathy and build rapport with diverse populations.

Preferred Experience\n

• At least one year of hands-on experience in working with children or families in a caregiving or support role is preferred.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 75,000 to 80,000 annually, commensurate with experience and qualifications.
