Work Environment\n

This position offers the flexibility of remote work while collaborating with an interdisciplinary team that includes healthcare providers, nurses, and caregivers. Our work hours are Monday through Friday, from 8:00 AM to 4:30 PM, though there may be occasional evening or weekend obligations depending on client needs. Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, considering emotional, mental, and social factors that affect their well-being.

• Develop personalized care plans in collaboration with clients and their families to ensure they receive appropriate support tailored to their unique situations.

• Provide counseling and emotional guidance, assisting seniors in managing life transitions, coping with loss, and navigating personal challenges.

• Act as an advocate for clients' rights and access to community resources, ensuring they are informed about various supports available to them.

• Coordinate with a multidisciplinary team, facilitating regular communication to enhance care delivery and address any emerging challenges or concerns.

• Perform regular home visits to monitor client conditions, assess safety, and provide immediate support as necessary.

• Educate clients and their families about available local services, including support groups, wellness programs, and activities aimed at enhancing social engagement.

• Maintain detailed and accurate documentation of assessments, care plans, progress notes, and other relevant client interactions in compliance with legal and ethical standards.

Qualifications\n

Compensation and Benefits\n
