Key Responsibilities\n

• Conduct thorough assessments of patients’ behavioral health to identify needs and appropriateness of care.

• Develop, implement, and manage personalized care plans tailored to the unique behavioral health requirements of each individual.

• Deliver engaging virtual psychotherapy sessions, each lasting between 45 to 50 minutes, utilizing effective therapeutic techniques.
\n
• Collaborate with integrated clinical teams to design and launch Water Edge Counseling’s comprehensive virtual-first cardiac recovery program.
\n
• Regularly evaluate and modify treatment plans based on patient progress and feedback, utilizing evidence-based practices to ensure optimal care.
\n
• Encourage patient engagement and enhance interaction through motivational interviewing and mindful counseling techniques.
\n
• Document patient interactions comprehensively and accurately, ensuring adherence to professional and regulatory standards.
\n
• Communicate effectively with team members and external health systems to ensure cohesive integration of care, especially in emergent situations.

Patient Population / Client Focus\n

You will primarily work with adult patients experiencing behavioral health challenges, particularly within cardiology. This position demands not only clinical expertise but also an empathetic approach to understand and connect with patients facing emotional and psychological barriers related to their health.

Qualifications\n

• A Bachelor’s Degree in Social Work is required; a Master's Degree is preferred.

• A current and valid LCSW license in the state of New York.

Compensation and Benefits\n
