Work Environment

The role is based onsite at our modern rehabilitation center located in Woodbridge, New Jersey. Here, you'll work closely with a multidisciplinary team within a welcoming environment that encourages transparency, collaboration, and personal growth. Our team operates within a flexible scheduling model of 40 hours a week, including some variable weekend shifts as needed.

Key Responsibilities

• Conduct thorough evaluations and assessments of patients to identify functional limitations and disabilities.
• Design, implement, and adapt individualized treatment plans based on patient evaluations, incorporating evidence-based practices.
• Collaborate with physicians and other healthcare providers, providing expert consultations concerning the patient's continuum of care.
• Educate patients and their families about injury prevention, rehabilitation exercises, and treatment goals, ensuring all communication is clear and empathetic.
• Maintain accurate and thorough documentation of patient progress, treatment plans, and discharge summaries in compliance with professional standards and regulatory requirements.
• Supervise and mentor support staff, ensuring effective treatment delivery and adherence to best practices.
• Participate in quality improvement initiatives and community outreach programs that align with our organizational goals.
• Engage in continuous self-improvement by attending in-services, workshops, and relevant educational opportunities.

Patient Population / Client Focus

As a Physical Therapist at Stone Crest, you will work with a diverse range of patients spanning various ages and cultural backgrounds. Our clientele consists primarily of individuals recovering from orthopedic surgeries, sports injuries, neurological disorders, and other conditions that impact mobility and function. You will have the opportunity to implement treatment plans that prioritize patient-centric approaches, adapting strategies to meet varied patient needs and enhancing their overall rehabilitation experience.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A valid State License in Physical Therapy.
• Exceptional written and verbal communication skills, allowing for effective interactions with patients, families, and interdisciplinary teams.
• Proficient computer and word-processing skills for documentation and patient management.
• Ability to perform physical tasks that involve lifting, repositioning, and assisting patients during treatment sessions.

Preferred Experience

• Minimum of 2 years of relevant physical therapy experience, particularly in outpatient rehabilitation settings.
• Demonstrated experience working collaboratively within a multidisciplinary team.
• Familiarity with electronic health records (EHR) systems to streamline documentation and patient management.

Compensation and Benefits

The compensation for this position is competitive, with a yearly salary range of USD 100,000 – 120,000, commensurate with experience and qualifications.
