Key Responsibilities\n

• Conduct thorough assessments to identify the needs and goals of each patient, creating tailored care plans to facilitate their progress.

• Maintain clear communication with supervisors, fellow counselors, and family members regarding patient progress and treatment plans.

• Establish and manage an organized schedule for appointments, home visits, and therapeutic observations to track the advancement of treatment plans.

• Coordinate and perform home visits, ensuring in-depth observational assessments and effective data collection.
\n
• Ensure that treatment plans are carried out efficiently and effectively, contributing directly to patient outcomes.
\n
• Participate in Interdisciplinary Team Meetings, leveraging collaboration to enhance treatment strategies and patient care.
\n
• Draft comprehensive progress notes every month and quarterly updates as required for patient records.
\n
• Organize and lead training sessions for staff to ensure competence in implementing treatment protocols and documentation guidelines.

• Provide follow-up support through interviews, home visits, and additional observations to ensure the continuous alignment of care with patient needs.

• Act as a liaison with relevant community stakeholders to bolster mental health initiatives and support development goals.

• Contribute to the development of training materials aimed at improving the efficacy of implemented programs for individuals.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) from the State of Arizona.
