Key Responsibilities\n

In your role as a Licensed Clinical Social Worker, you will be responsible for:

• Conducting comprehensive assessments to develop tailored treatment plans for hospice patients dealing with end-of-life issues.

• Providing individual and family therapy sessions, focusing on grief and loss, emotional distress, and coping strategies.
\n
• Facilitating communication between patients, families, and caregivers to ensure a holistic support system and resource accessibility.

• Assisting families in managing complex emotions associated with terminal illnesses, including anticipatory grief and feelings of loss.

• Collaborating with medical staff to integrate behavioral health into the overall care plan, ensuring that emotional and psychological needs are met.

• Maintaining detailed and accurate documentation within electronic medical records (EMR), contributing to comprehensive care coordination.

• Participating in weekly clinical meetings to discuss case progress and share insights with fellow team members.

Patient Population / Client Focus\n

This position will primarily focus on working with individuals who are in hospice care, along with their families. You will address a variety of issues, including:

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Bachelor's degree in Social Work (BSW) or a related field is required.

• An active and unrestricted LCSW license in the state of Colorado.

• A minimum of 2 years of clinical experience, particularly in hospice, palliative care, or similar settings.

• Strong interpersonal skills and the ability to build rapport quickly with patients and families.

• Compassionate, patient-oriented approach with excellent clinical judgment.

• Proficiency in electronic health record (EHR) systems and telehealth platforms.

Preferred Experience\n

Compensation and Benefits\n
