Key Responsibilities\n

• Conduct comprehensive bio-psychosocial assessments to evaluate mental health, substance abuse, and medical needs, ensuring that assessments are culturally and age-appropriate.

• Deliver behavioral health services to individuals, couples, and families, which encompass counseling, intake assessments, diagnosis, treatment planning, and crisis intervention.

• Document clinical interactions electronically, including mental health evaluations, progress notes, and suicide risk assessments, maintaining compliance with privacy regulations.
\n
• Facilitate referrals to local community resources and services, providing social work support and making connections to essential support systems, including medication assistance programs.

• Collaborate with Quality Improvement teams to implement data collection processes, enhancing service delivery and patient outcomes.

Qualifications\n

• A Master’s degree in Social Work (MSW) is required.

• Must hold a current and valid Licensed Clinical Social Worker (LCSW) license in the state of California.
