This position is fully remote, providing you with the flexibility to work from home while delivering critical support to our patients at the Hospice Center in Visalia, California. Key Responsibilities\n

• Conduct comprehensive assessments of patients to identify emotional and behavioral issues and develop tailored intervention plans.

• Provide crisis intervention services, helping patients manage acute psychological distress associated with their health circumstances.

• Regularly communicate with the healthcare team, including physicians and nursing staff, to update them on patient progress, address concerns, and discuss treatment efficacy.

• Document clinical notes accurately and timely, adhering to departmental policies to ensure compliance and continuity in patient care.

• Collaborate with other healthcare professionals in developing discharge plans that are informed by psychosocial assessments and best practice social work methodologies.

• Establish and maintain professional relations with community resource groups, serving as a representative of Inner Peace Counseling Center in various committees aimed at addressing patient social needs.

Patient Population / Client Focus\n

You will primarily work with patients in hospice care, focusing on providing emotional support to individuals facing end-of-life issues and their families. Qualifications\n

• Master's degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology is mandatory.

• Active licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT) in California.

• A minimum of 3 years of professional experience in a mental health counseling role is required.
