Work Environment

The position is primarily onsite, requiring you to travel to patients' homes throughout Leesburg and the surrounding areas. Our home care model allows you to provide personalized attention and interventions in a comfortable setting for the patient, ensuring they receive the best rehabilitation without the constraints of traditional clinic hours. You will work a standard schedule from 9 am to 5 pm, Monday to Friday, allowing for a healthy work-life balance.

Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions and develop individualized care plans that reflect their unique rehabilitation goals.
• Deliver tailored physical therapy treatments using a variety of techniques, including manual therapy modalities, exercise programs, and patient education.
• Monitor patients' progress, adjusting treatment plans as necessary to ensure optimal results and adherence to evidence-based practices.
• Document all patient interactions and therapy sessions accurately in our electronic medical record system, with a focus on thoroughness and compliance.
• Collaborate with multidisciplinary teams, including physicians, occupational therapists, and other healthcare providers, to ensure holistic care for your patients.
• Educate patients on how to manage their conditions effectively and promote a lifestyle conducive to healing and improvement.
• Maintain a manageable patient load to ensure that each individual receives the care and attention they deserve, fostering strong therapeutic relationships.

Patient Population / Client Focus

As an Orthopedic Physical Therapist, you will primarily work with patients recovering from orthopedic surgeries, sports injuries, chronic pain conditions, and those seeking rehabilitation following musculoskeletal trauma. Your expertise will contribute significantly to improving their physical function, pain management, and overall quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy or a relevant field; Doctorate in Physical Therapy (DPT) preferred.
• Active Physical Therapist license in Virginia, with no history of sanctions or board proceedings.
• Proficient in a variety of manual therapy techniques and exercise programming tailored to individual patient needs.
• Demonstrated ability in utilizing EMR software, specifically Web PT, for documentation and patient tracking.
• Strong understanding of up-to-date physical therapy practices backed by current research and clinical evidence.

Preferred Experience

• Previous experience in outpatient orthopedic settings or home health environments.
• Proven track record of developing successful rehabilitation programs and improving patient outcomes.
• Exceptional communication and interpersonal skills for fostering patient relationships.
