Key Responsibilities\n

• Identify risk factors and implement screening criteria for patients needing special precautions, such as those exhibiting suicidal or homicidal ideations.

• Conduct thorough evaluations and assessments, ensuring timely and accurate documentation that complies with regulatory standards.

• Collaborate closely with a multidisciplinary team to assess patient needs and coordinate effective discharge planning, ensuring readiness for transition.

• Facilitate follow-up consultations post-discharge to monitor patient progress and address any ongoing concerns.

• Lead one-on-one, group, and family psychoeducational sessions, utilizing evidence-based therapeutic strategies.

• Develop, review, and revise personalized treatment plans in conjunction with treatment planning meetings, maintaining an updated focus on patient goals.

• Assess patient environments for safety and proactively address any emerging safety concerns or changes in clients' mental health conditions.

Qualifications\n
