Key Responsibilities

• Conduct age and culturally appropriate comprehensive bio-psychosocial assessments for clients presenting with mental health and substance use issues within a primary care setting.
• Deliver a range of behavioral health services to individuals, couples, and families, including intake assessments, therapeutic counseling, crisis intervention, and treatment planning.
• Create and maintain electronic documentation of mental health evaluations, progress notes, and treatment plans, ensuring compliance with legal and ethical standards.
• Perform suicide risk assessments and follow up with necessary healthcare providers to enhance patient safety and response.
• Serve as a liaison to connect clients with community resources and social services, including medication assistance programs, to support holistic health outcomes.
• Collaborate actively with the Quality Improvement teams to enhance data collection processes, contributing to the clinic’s continuous quality improvement initiatives.

Qualifications

Preferred Experience

Compensation and Benefits
