Key Responsibilities\n

• Conduct comprehensive assessments to identify the specific needs and goals of each patient, utilizing both standardized tools and personal interviews.

• Engage in ongoing communication with supervisors, direct care staff, family members, and other stakeholders to provide updates on individual progress and treatment plans.

• Develop structured schedules for counseling appointments and home visits to monitor and support patient progress effectively.

• Coordinate and undertake home visits, performing in-depth observations and chart reviews to ensure comprehensive treatment.

• Facilitate the implementation of tailored program plans, ensuring each strategy is both effective and appropriately executed for each patient.

• Participate actively in interdisciplinary team meetings to contribute insights and strategies for improved patient outcomes.

• Document monthly and quarterly progress notes that track patient development and modify treatment plans as necessary.

• Organize and lead training sessions to certify staff in the delivery of program plans and adherence to documentation protocols.

• Conduct follow-up interviews and additional home visits to ensure continuous support and adaptation to patient needs.

• Maintain meticulous records, applying a focus on accessibility and compliance for interested parties.

• Act as a liaison with the public and mental health community to further our developmental goals and outreach efforts.

• Contribute to the creation of procedures and training materials designed to enhance our counseling programs for individuals and families.

Qualifications\n
