Key Responsibilities\n

• Engage in continuous communication with physicians and healthcare staff to provide updates on patient progress, specific needs, concerns, and any potential issues that arise during treatment.

• Maintain accurate and timely documentation, including clinical notes and progress reports, in accordance with departmental policies and compliance standards.
\n
• Collaborate with the multidisciplinary healthcare team to create individualized treatment plans that include focused discharge planning based on thorough psychosocial assessments.

• Provide counseling services and crisis intervention support to clients experiencing emotional and behavioral challenges, employing appropriate assessment and therapeutic techniques.

• Facilitate discharge planning and care coordination by working closely with clients and various healthcare providers, ensuring seamless access to ongoing services.

• Consult with clients’ families and support systems regarding treatment approaches and discharge planning, emphasizing the importance of a supportive and nurturing environment.

Patient Population / Client Focus\n

In this role, you will predominantly serve adults facing a variety of mental health issues, including anxiety, depression, trauma, and substance use disorders. Qualifications\n

• A Master’s degree in Social Work, Counseling, or Marriage and Family Therapy is mandatory for this role.
