Key Responsibilities\n

• Conduct comprehensive assessments to identify seniors' emotional, mental, and social needs, ensuring a holistic approach to their care.

• Develop tailored care plans with clients and their families that address their specific goals, preferences, and potential challenges.

• Facilitate counseling sessions to provide emotional support, helping clients navigate life transitions and challenges associated with aging.

• Advocate for clients' rights by connecting them to vital community resources and services necessary for their well-being.

• Collaborate with a diverse team of professionals—including healthcare providers, caregivers, and community organizations—to coordinate comprehensive care.

• Conduct periodic visits, either in-person or via video conferencing, to assess clients’ living conditions, monitor their mental health, and address any safety concerns.

• Educate clients and their families on available programs, support groups, and community resources designed to enhance their social engagement.

• Document all interactions meticulously, including assessment results, care plans, and progress notes, ensuring compliance with all regulatory and ethical standards.

• Stay current with best practices and evolving trends in social work and aging services, maintaining your licensure and professional development.

In this role, you will primarily work with seniors, providing them with the support and resources they need as they navigate various life stages. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• A valid state licensure as a Licensed Master Social Worker (LMSW) is essential.

Compensation and Benefits\n
