This position allows you to work remotely, providing flexibility while ensuring you have the resources and support needed to succeed. Our collaborative virtual environment encourages regular communication with an interdisciplinary team—including nurses, caregivers, and healthcare professionals—aimed at delivering comprehensive care to clients. A typical workweek runs from Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client needs.

Key Responsibilities\n

• Conduct thorough psychosocial assessments of seniors, taking into account their emotional, mental, and social needs.

• Collaborate with clients and their families to develop individualized care plans, ensuring alignment with their personal goals.
