This full-time role provides the flexibility of remote work while engaging with a diverse client base through virtual appointments and telehealth platforms. Although the primary work setting is remote, our Social Workers are integrated into a larger interdisciplinary team within our Hospice Center, collaborating closely with healthcare professionals to ensure coordinated and effective care.

Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand clients' mental health challenges, strengths, and available resources.

• Provide individual and family therapy sessions as well as facilitate group therapy, creating a safe space for expression and healing.

• Implement evidence-based treatment modalities tailored to the needs of clients, including cognitive-behavioral therapy and trauma-focused interventions.

• Develop comprehensive and individualized treatment plans that outline therapeutic goals and strategize interventions.

• Collaborate with psychiatrists, psychologists, and the overall treatment team to ensure well-coordinated, holistic care for each client.

• Offer crisis intervention, including risk assessments and safety planning, particularly in high-stress situations.

• Maintain accurate and timely documentation of client interactions in compliance with legal and ethical requirements.

• Facilitate psychoeducation sessions and support groups to empower clients and enhance their resilience in managing their mental health.

Qualifications\n

Compensation and Benefits\n
