Key Responsibilities\n

• Conduct thorough assessments and personalized mental health evaluations to understand each client's needs and develop treatment plans.

• Maintain open lines of communication between supervisors, clients, families, and other stakeholders about patient progress and treatment interventions.

• Provide a regular schedule for appointments, home visits, and necessary observations to ensure the effective implementation of treatment and monitoring of patient progress.

• Facilitate and coordinate home visits to meet the requirements of individual patient care, which may include chart reviews and staff interviews.

• Ensure the effective execution of treatment plans and monitor their impact on individual patient outcomes.

• Participate actively in Interdisciplinary Team Meetings to discuss treatment approaches and assess patient progress collaboratively.

• Complete and document monthly and quarterly progress notes to track patient development and adjust care needs accordingly.
\n
• Organize training sessions to ensure that all team members are equipped with the competencies needed to deliver high-quality services.
\n
• Conduct follow-up interviews, home visits, and observations to continue providing tailored support based on client needs.
\n
• Maintain comprehensive records to ensure that all relevant information is accessible for staff and involved parties.

• Act as a liaison between the organization and the wider mental health community to facilitate goal-focused development.

• Assist in developing procedures and training materials aimed at enhancing team capabilities and patient care programs.

Patient Population / Client Focus\n

You will work with a diverse range of clients, including children, adolescents, and adults, who present with various mental health challenges such as anxiety, depression, trauma, and relationship issues. Your expertise in specialized areas like family therapy or substance misuse will play a crucial role in addressing the unique needs of each patient, ensuring that they receive the most appropriate and effective care.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in Arizona.
