This full-time role offers a competitive salary range of USD 80,000 to 105,000 annually, depending on experience and caseload. Key Responsibilities\n

• Conduct comprehensive assessments that identify client needs and tailor treatment plans accordingly.

• Maintain clear and continuous communication with supervisors, direct care staff, family members, and other stakeholders regarding client progress.

• Develop and provide a structured schedule for various client appointments, home visits, and monitoring activities.
\n
• Carry out detailed evaluations during home visits, ensuring compliance with observational requirements and thorough documentation through chart reviews and data analysis.
\n
• Implement and evaluate program plans, ensuring they align with best practices and achieve intended outcomes.
\n
• Engage in Interdisciplinary Team Meetings, contributing insights and strategies to enhance the overall care provided.
\n
• Document client progress through detailed monthly and quarterly notes, fostering a clear understanding of developments and treatment effectiveness.
\n
• Organize necessary training sessions to ensure all team members adhere to and understand the program plan and documentation standards.
\n
• Provide ongoing follow-up to clients through interviews and observations as part of their continued care and support.
\n
• Act as a liaison between the organization and the broader mental health community, helping to foster developmental goals.

Patient Population / Client Focus\n

In this role, you will engage with a diverse client base, including both youth and adults who are coping with various challenges, including mental health disorders, family conflicts, and substance misuse. Your expertise will be critical in providing appropriate therapeutic interventions, fostering trust and connection, and facilitating positive change in their lives.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of active, unrestricted licensure as a Professional Counselor in the state of Colorado.

• A minimum of two years of experience in counseling, ideally in a clinical setting with exposure to a range of client issues.

• Proficiency with Electronic Medical Records (EMR) software and related technologies is essential for effective documentation and client management.

• Demonstrated ability to communicate effectively and establish rapport with clients of diverse backgrounds, fostering an environment of trust and openness.
