Key Responsibilities\n

• Conduct thorough assessments to understand the unique needs of each client, ensuring tailored mental health interventions.

• Maintain open communication with supervisors, direct care staff, family members, and relevant personnel regarding patient progress and treatment adjustments.

• Develop and provide a schedule of appointments, including in-home visits, and monitor the treatment advancement effectively.

• Coordinate and conduct home visits, fulfilling the criteria for in-home observations and comprehensive data gathering.

• Implement individualized program plans to ensure the effective delivery of therapeutic techniques to clients.

• Participate actively in interdisciplinary team meetings to discuss treatment plans, patient progress, and approaches to improve care delivery.

• Document patient progress in monthly and quarterly reports, utilizing electronic medical record (EMR) systems to maintain comprehensive client history.
\n
• Organize and conduct training sessions to enhance team competence in implementing treatment plans and documentation practices.
\n
• Provide ongoing follow-up through evaluations and observations to support continued patient needs.
\n
• Act as a liaison to the broader mental health community, facilitating resource access and support for developmental goals.

• Fine-tune procedures for training materials that enhance program effectiveness for clientele.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• An active and unrestricted license as a Professional Counselor in the state of Texas is mandatory for this position.
