Key Responsibilities\n

• Conduct thorough ongoing assessments to evaluate the mental health needs of clients and formulate individualized treatment plans.

• Communicate effectively with various stakeholders, including supervisors, care teams, family members, and guardians to discuss patient progress and adjustments to treatment plans.

• Provide a structured schedule of appointments, home visits, and observations to supervisors, ensuring that all necessary monitoring tasks are completed.
\n
• Coordinate and oversee essential home visits to fulfill program requirements, which include in-home observations, chart reviews, and staff consultations.
\n
• Ensure the integrity of the program implementation by monitoring compliance with treatment protocols and making adjustments as needed.
\n
• Participate in Interdisciplinary Team Meetings to address complex cases collaboratively and share valuable insights regarding client care.
\n
• Document monthly and quarterly progress notes detailing client advancements, therapeutic interventions, and any significant changes.
\n
• Organize and deliver training sessions to certify team members on the implementation of the program plan, ensuring optimal documentation practices.
\n
• Conduct follow-up interviews and home visits to provide continued support and assess clients’ evolving needs.
\n
• Maintain thorough and accurate records of client interactions and outcomes, readily available for both internal and external stakeholders.

• Act as a liaison within the mental health community to promote essential development goals and resources for clients.

• Assist in creating and updating training materials to optimize programs aimed at enhancing client outcomes.

Patient Population / Client Focus\n

Our LPC will engage with a diverse client base, including both adults and youth, who present a variety of mental health challenges. Qualifications\n

• A Master's degree or Doctoral degree in Counseling, Psychology, or a related field is required.
