Work Environment

Key Responsibilities

• Perform comprehensive health assessments, physical examinations, and screenings for individuals and families, utilizing diagnostic tools and advanced medical equipment.
• Diagnose and effectively manage both acute and chronic medical conditions, ensuring patients receive preventive care and health promotion tailored to their circumstances.
• Work collaboratively with community organizations, schools, and local agencies to establish and implement health education programs that promote wellness and disease prevention.
• Exercise cultural competency while providing care, maintaining a strong advocacy for patients' rights and help navigating the healthcare system.
• Facilitate referrals and coordinate follow-up care, ensuring that patients receive comprehensive and continuous medical support.

Patient Population / Client Focus

Qualifications

• Current certification as a Nurse Practitioner, with advanced clinical training in family medicine.
• Active, unrestricted state nursing license in New York.

Compensation and Benefits
