Key Responsibilities\n

• Utilize electronic medical records (EMR) systems to monitor, document, and communicate patient progress towards treatment goals and the development of personalized social work plans.

• Engage in collaborative discussions with an interdisciplinary team to anticipate patient needs and facilitate the advancement of care plans.

• Provide comprehensive support and resources to patients and their families regarding transition plans, ensuring a smooth and successful service transfer.

• Maintain an up-to-date knowledge of governmental and private payer networks to guarantee appropriate patient transitions.

• Collaborate with community resources and healthcare organizations to coordinate care tailored to individual patient needs.
\n
• Advocate for patients and their families, ensuring their rights are upheld and access to protective services is available.
\n
• Demonstrate a deep understanding of ethical decision-making and patient rights, providing guidance for both patients and families.

• Ensure appropriate reporting of any suspected abuse or vulnerability as mandated by state law.

• Engage in care system processes aimed at preventing patient readmissions, utilizing data and assessments to identify high-risk patients.
\n
• Organize and participate in transition conferences, facilitating effective dialogue among patients, families, and care teams.
\n
• Implement technological tools to identify at-risk patients and intervene proactively.
\n
• Facilitate thorough clinical handoffs for at-risk patients, which may include essential referrals to appropriate services.

• Perform other duties as assigned that align with the organization's goals.

• Offer emotional support and counseling to patients and their families, providing valuable resources for identified social or financial needs.

Patient Population / Client Focus\n

Your primary focus will be on patients navigating various psychological challenges related to both acute and chronic conditions, often including mental health disorders. You will support diverse demographics and foster a therapeutic relationship built on trust and empathy.

Qualifications\n

• A valid Master’s or Doctorate degree in clinical psychology, counseling, or clinical social work from an accredited institution (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).

• An independent license in your state of residence (e.g., LCSW, LMFT, LPC) is mandatory.

• Strong capacity to manage a substantial caseload while effectively interfacing with multi-disciplinary teams and support staff.

• Proficient skills in utilizing electronic medical records and efficient communication via electronic mail.

• Annual salary range of USD 100,000 – 105,000.
