You'll care for patients recovering from illness, surgery, injury, or chronic medical conditions while helping them remain independent in the comfort of their own homes.

Rather than seeing patients for only a few minutes, you'll have the opportunity to provide thoughtful, individualized care while becoming a trusted member of each patient's healthcare team.

What You'll Be Doing

• Provide skilled home health nursing visits throughout your assigned territory
  
• Complete comprehensive patient assessments
  
• Perform wound care , medication management, injections, and other skilled nursing services
  
• Monitor patient progress and communicate changes to physicians
  
• Educate patients and caregivers on disease management and treatment plans
  
• Coordinate care with therapists, social workers, and interdisciplinary team members
  
• Complete timely documentation within our Homecare Homebase (HCHB) EMR
  
• Help reduce hospitalizations through proactive, high-quality patient care
  

Who We're Looking For

• Active Minnesota Registered Nurse (RN) license
  
• Previous nursing experience in home health, hospital, rehabilitation, skilled nursing, or clinical care
  
• Strong assessment and critical thinking skills
  
• Ability to work independently while collaborating with an interdisciplinary team
  
• Excellent communication and organizational skills
  
• Reliable transportation and willingness to travel throughout the local service area
