Pride Health is hiring a Physical Therapist (PT) - Home Health to support our client's Home Health Team in NORTH EASTON, MA.


Job Details:
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Location: North Easton, MA 02356
Float / Coverage Area: Dedham, Braintree, Quincy, and surrounding communities.
Job Type: Local contract
Contract Length: 14+ Weeks
Pay Range: $50.00 to $57.00/hour*
Mileage Reimbursement Rate (if applicable) : $0.52/mile
*Pay offered is based on experience, expertise, credentialing, and education.


Duties:

• Perform comprehensive physical therapy evaluations and assessments in the home setting.
• Develop individualized Plans of Care (POC) based on physician orders, patient needs, and clinical findings.
• Deliver evidence-based therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence.
• Conduct reassessments and modify treatment plans as needed to achieve optimal patient outcomes.
• Supervise and mentor Physical Therapist Assistants (PTAs), ensuring treatment plans are implemented appropriately and in compliance with regulatory standards.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, caregivers, and interdisciplinary team members to coordinate patient care.
• Educate patients and families on therapeutic exercises, fall prevention, home safety, mobility training, and disease management strategies.
• Complete accurate and timely clinical documentation while maintaining productivity expectations.

Requirements:

• Active Massachusetts Physical Therapist (PT) License in good standing.
• Graduate of an accredited Physical Therapy program.
• Previous Home Health experience required .
