Key Responsibilities\n

As an LCSW, your responsibilities will include:

• Conducting thorough bio-psychosocial evaluations to identify and address barriers preventing patients from achieving their highest levels of functioning.

• Developing and implementing individualized treatment plans that draw on evidence-based methodologies and therapeutic interventions.

• Documenting all social work activities and patient interactions in compliance with departmental policy and ethical standards.

• Consulting with medical and support staff to ensure a coordinated approach to patient care, enhancing treatment compliance and satisfaction.

• Providing education and support to patients and their families on understanding and managing their conditions.

• Assessing advocacy needs and coordinating resources to empower patients within their communities.

Patient Population / Client Focus\n

This role primarily focuses on patients in hospice care, dealing with a variety of psychosocial issues related to terminal illnesses and end-of-life concerns. Qualifications\n

The ideal candidate for this position will possess:

• A Master’s Degree in Social Work from an accredited institution.

Compensation and Benefits\n

Clear Sky Family & Marriage Counseling PC offers a competitive salary range of USD 85,000 – 100,000 per year, commensurate with experience and qualifications.
