Key Responsibilities\n

• Conduct initial assessments with clients to identify their goals, preferences, and available support systems, establishing a foundational understanding that informs their personalized rehabilitation strategies.

• Develop and implement tailored rehabilitation plans that consider individual educational backgrounds, unique skills, physical abilities, and vocational aspirations, making modifications as necessary to ensure progress and relevance to the client's evolving needs.

• Collaborate closely with a multidisciplinary team consisting of physicians, occupational therapists, psychologists, and other healthcare professionals to deliver integrated rehabilitation programs specifically created for each client's needs.

• Maintain meticulous case files that document relevant client information, including personal history, eligibility, insurance details, services rendered, and ongoing communication, ensuring accurate records for compliance and quality assurance.

• Monitor each client’s progress diligently, documenting their achievements and challenges regularly, and using this information to adjust treatment plans for optimal outcomes.

• Engage with clients throughout their participation in job training, occupational placement programs, and supplementary activities, proactively addressing any placement challenges that arise.

• Work collaboratively with the families and support networks of clients to develop effective strategies tailored to address their needs.
\n
• Conduct thorough needs assessments and provide timely documentation to facilitate appropriate referrals and resources.

Patient Population / Client Focus\n

Our primary focus is on individuals experiencing a variety of mental health conditions, including mood disorders, anxiety, trauma-related symptoms, and other psychological challenges. You will work one-on-one with clients, empowering them to improve their quality of life and achieve their therapeutic goals.

Qualifications\n

• A master’s degree in counseling, social work, psychology, or a related field is required.

• A current Licensed Professional Counselor (LPC) credential.
\n
• Proven experience in mental health treatment, particularly with individuals facing mental illness and mood disorders in both inpatient and outpatient settings.

• A collaborative mindset that prioritizes teamwork and shared success in achieving meaningful client change.

• Exceptional clinical skills, including proficiency in techniques such as motivational interviewing, cognitive-behavioral therapy (CBT), and dialectical behavior therapy (DBT), accompanied by thorough documentation practices.
