Key Responsibilities

• Conduct comprehensive physical therapy evaluations to assess residents' mobility, strength, and functional capabilities, tailoring services to meet individual needs.
• Develop and implement personalized treatment plans focused on pain management, improving mobility, and preventing falls, ensuring each plan is aligned with residents' specific goals.
• Administer hands-on therapeutic interventions, including a range of manual therapy techniques, therapeutic exercises, and balance training to promote recovery.
• Utilize and recommend appropriate assistive devices, such as walkers or canes, to foster residents' independence and enhance their daily living activities.
• Collaborate closely with nursing staff, occupational therapists, and caregivers to ensure a seamless continuum of care, sharing insights and progress updates during inter-disciplinary meetings.
• Educate residents and their families about home exercise programs, effective mobility strategies, and safety precautions to foster confidence in their rehabilitation journey.
• Maintain accurate and timely documentation in compliance with skilled nursing facility regulations and standards, ensuring consistency in resident care and progress reporting.

Patient Population / Client Focus

This position is focused primarily on a geriatric clientele, many of whom are recovering from surgeries, managing chronic illnesses, or facing mobility challenges associated with aging. Your efforts will significantly contribute to enhancing their quality of life, promoting independence, and enabling them to engage more fully in their daily activities.

Qualifications

• Active and valid Physical Therapy license in Virginia.
• A Doctor of Physical Therapy (DPT) or Bachelor's/Master's degree in Physical Therapy from an accredited program.

Preferred Experience

• At least two years of previous experience in a Skilled Nursing Facility (SNF) or geriatric rehabilitation setting, focusing on providing care that meets the unique needs of older adults.
• Familiarity with long-term care regulations and knowledge of evidence-based rehabilitative practices is highly advantageous.

Compensation and Benefits
