Key Responsibilities\n

• Utilize your clinical training to independently assess and diagnose patients’ psychosocial conditions through detailed evaluations.

• Develop collaborative and patient-centered treatment plans that cater to individual needs and are aligned with evidence-based practices.

• Implement therapeutic interventions, such as solution-focused therapies, motivational interviewing, and problem-solving techniques tailored to support clients through challenging times.

• Engage in interdisciplinary collaboration with psychologists, primary care providers, and other service providers to offer holistic care, ensuring a comprehensive approach to each patient’s mental health needs.

• Monitor patient progress systematically by tracking treatment responses and adapting approaches based on evidence gathered during sessions, whether conducted in person or virtually.

• Assist in managing psychotropic medications, focusing on adherence to prescribed treatments, monitoring side effects, and evaluating overall treatment effectiveness, in coordination with medical providers.

Patient Population / Client Focus\n

You will primarily serve patients in a hospice setting, addressing their emotional, psychological, and social needs as they navigate the complexities of terminal illness and end-of-life care. This role requires sensitivity and empathy, as well as the ability to support patients and their families with compassion and understanding during one of life’s most difficult transitions.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current, valid, and unrestricted LCSW license in the state of Georgia.

• At least one year of verified experience working independently in a therapeutic setting.
