Key Responsibilities\n

• Utilize your education and professional training to independently assess and diagnose psychosocial conditions in hospice patients, tailoring your approach to individual needs.

• Engage patients in developing personalized treatment plans that address their unique emotional, psychological, and social needs.

• Implement evidence-based behavioral interventions such as solution-focused therapy, motivational interviewing, and behavioral activation, ensuring each intervention aligns with patient goals.

• Coordinate care with medical professionals, including physicians and psychologists, to integrate psychosocial support with physical healthcare services, fostering a comprehensive approach.

• Regularly monitor and document treatment responses, using both in-person interactions and virtual sessions to track progress toward achieving care objectives.

• Support patients in medication management, addressing issues related to adherence, side effects, and treatment effectiveness as prescribed by the medical team.

Qualifications\n

Compensation and Benefits\n
