Key Responsibilities\n

• Conduct in-depth evaluations and assessments to formulate comprehensive treatment plans tailored to individuals and families coping with significant life transitions.

• Deliver intensive counseling and psychotherapy services, focusing on grief, loss, and emotional distress, while applying evidence-based therapeutic models.

• Perform thorough mental status exams, identifying signs of distress, and adapting interventions accordingly to improve client outcomes.

• Maintain accurate and timely documentation of case notes, progress reports, and treatment plans to ensure continuity of care and compliance with regulatory standards.

• Effectively manage high-stress scenarios with a calm, empathetic approach, utilizing de-escalation techniques and therapeutic communication skills to support clients in crisis.

• Engage in interdisciplinary collaboration with psychiatrists, case managers, and other healthcare professionals to coordinate comprehensive care plans that address the complexities of clients’ needs.

• Facilitate individual and group therapy sessions, adjusting techniques as necessary based on client feedback and evolving therapeutic goals.

• Actively participate in case consultations, sharing insights and strategies to foster a culture of learning and continuous improvement within the team.

Patient Population / Client Focus\n

In this role, you will primarily focus on patients facing terminal diseases and their families, addressing the profound emotional, psychological, and social challenges that arise during the hospice experience. You will work closely with individuals experiencing grief and loss, offering therapeutic support that allows clients to navigate their feelings, fostering resilience, and facilitating healthier coping mechanisms. Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Current licensure in Maryland as a Licensed Clinical Social Worker (LCSW-C) is mandatory, along with the capability to practice independently.
