Key Responsibilities\n

• Utilize your extensive training and experience to formulate independent psychosocial diagnoses for patients, based on comprehensive assessments.

• Collaborate with patients to create individualized, patient-centered treatment plans that address their unique mental health challenges and goals.

• Implement a variety of evidence-based behavioral interventions, such as solution-focused treatment, motivational interviewing, and problem-solving techniques to foster positive outcomes.

• Coordinate care with a multidisciplinary team, including psychologists and primary care providers, to ensure an integrated approach that addresses both physical and mental health needs.

• Track and assess treatment responses through regular monitoring of patient progress, conducting assessments either in person or through virtual platforms.

• Provide support in psychotropic medication management in accordance with healthcare providers' prescriptions, focusing on treatment adherence, side effects, and overall effectiveness.

Patient Population / Client Focus\n

You will primarily work with patients who are in need of hospice care, a sensitive area where mental health support is crucial. The population includes individuals facing end-of-life transitions, requiring not only psychological support but also assistance in family dynamics, grief counseling, and navigating complex emotional landscapes. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current, valid, and unrestricted license to practice independently in mental health as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee.
