Key Responsibilities\n

• Utilize your education and training to conduct comprehensive psychosocial assessments for patients, identifying their emotional, social, and environmental needs.

• Collaborate with patients to create individualized, patient-centered treatment plans that promote recovery and comprehensive well-being.

• Implement evidence-based interventions such as solution-focused therapy, behavioral activation, problem-solving treatment, and motivational interviewing to aid clients in overcoming their challenges.

• Engage with other healthcare professionals within our network to provide an integrated approach to care that addresses both mental health and substance use disorders.

• Monitor patient progress towards treatment objectives systematically, utilizing both in-person and virtual care methods to ensure accountability and support.

• Assist in managing psychotropic medications as prescribed, focusing on monitoring adherence, potential side effects, and overall treatment efficacy.

Patient Population / Client Focus\n

The clientele at Elevate Care Consultants primarily consists of individuals admitted to our Detox Center who are experiencing substance use disorders. These patients often face complex Co-occurring mental health conditions and require compassionate, skilled social work intervention to navigate their recovery journey effectively. As a social worker, you will play a critical role in helping these clients reclaim their lives by providing the support and guidance needed for sustained healing.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• A current and valid LCSW license to practice independently in the State of Maryland or the territory where you will provide services is essential.
