Work Environment

This full-time position is primarily onsite, catering to home care settings in a variety of local neighborhoods. You will work Monday to Friday, where your schedule will allow for flexibility and adaptability to meet patient needs. This role also offers opportunities for community outreach and involvement in marketing activities.

Key Responsibilities

• Conduct comprehensive evaluations of patients, including the review of medical histories, performance of detailed assessments, and documentation of findings to develop individualized treatment plans.
• Clearly communicate evaluation outcomes and treatment strategies to both patients and their families, ensuring collaboration in goal-setting processes.
• Implement best practices in delivering care, adjusting treatment protocols based on patient feedback and emerging healthcare trends.
• Regularly consult with other healthcare professionals to optimize patient care and make necessary modifications to care plans based on evolving patient conditions.
• Manage discharge planning effectively, ensuring patients have a smooth transition post-treatment and providing clear follow-up processes.
• Maintain accurate documentation and billing related to patient care actions, ensuring compliance with healthcare regulations and standards.
• Exemplify effective time management skills and occasional participation in departmental meetings and community engagement events.
• Engage in self-directed professional development, pursue continuing education opportunities, and serve as a mentor for student clinical interns.
• Promote an excellent patient experience through empathetic care and a commitment to access improvement initiatives.

Patient Population / Client Focus

You will focus on a diverse patient population that may include individuals recovering from surgeries, strokes, or managing chronic conditions. Understanding the distinctive needs of homebound patients in the Sayreville area is essential, as you will be working in a familiar environment for the patients, which can significantly enhance their comfort and recovery journey.

Qualifications

• Doctorate in Physical Therapy (DPT) with a valid state license to practice in New Jersey.
• Current CPR certification through the American Heart Association (BLS for Healthcare Providers).
• Strong clinical skills with demonstrated ability to work collaboratively in a team-centered environment.
• Attention to detail and commitment to delivering high-quality patient care.

Preferred Experience

• A minimum of 2 years of relevant clinical experience is preferred but not required.

Compensation and Benefits
