Work Environment

This full-time position is based onsite, allowing for meaningful interaction with patients directly in their homes. The work schedule is flexible, typically structured as Monday through Friday for a total of 40 hours a week. Key Responsibilities

• Deliver exceptional patient care by evaluating, planning, and implementing individualized treatment programs for patients requiring physical therapy in their homes.
• Conduct thorough evaluations that include gathering medical histories, administering physical assessments, and determining the most effective treatment strategies to achieve optimal outcomes.
• Document all patient interactions meticulously, including initial evaluation findings, treatment plans, progress notes, and discharge summaries to ensure clear communication within the care team.
• Collaborate with interdisciplinary teams and maintain regular communication with patients, their families, and referring physicians, fostering a strong support network for recovery.
• Stay ahead of changing regulations by completing required training and successfully upholding compliance with state and federal laws governing physical therapy practice.
• Engage in facility meetings and contribute insight for continuous improvement initiatives within the organization.

Patient Population / Client Focus

In this role, you will primarily work with a diverse patient population, including individuals recovering from orthopedic surgery, injury rehabilitation, and chronic pain management. Your expertise will be pivotal in tailoring treatment modalities that address each patient's unique needs, conditions, and therapeutic goals.

Qualifications

• A Doctorate Degree in Physical Therapy (DPT) from an accredited institution is required.
• A valid physical therapy license in the state of Virginia is essential.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

• At least two years of experience as a practicing physical therapist is preferred, including experience in home healthcare settings.
• Demonstrated ability to develop and execute comprehensive treatment plans based on evidence-based practices.
