Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of clients to determine their needs and eligibility for services.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), ensuring families are engaged at every stage.

• Monitor and evaluate progress towards treatment goals while adjusting recommendations as necessary to best support the child and family.

• Provide valuable support and education to families about child development, drawing from best practices and tailoring strategies to individual needs.
\n
• Utilize evidence-based practices, particularly emphasizing Natural Learning Environment practices to enhance children's skills through everyday situations.
\n
• Develop strong rapport with children and their families, fostering trust and a supportive atmosphere that encourages growth and learning.
\n
• Navigate and comply with all federal and state regulations governing social work practice, ensuring organizational standards for health and safety are upheld.

• Document all interactions and interventions clearly and accurately in the relevant databases and systems to maintain compliance and enhance continuity of care.

Patient Population / Client Focus\n

The primary focus of this position is on children with disabilities or developmental delays, along with their families. You will engage with families to provide early intervention services, aimed at supporting their child's development in areas such as communication, cognitive skills, and emotional regulation. As a social worker within a hospice setting, your role also involves compassionate engagement with families during sensitive times of palliative and end-of-life care.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) credential issued by the Department of Public Health.

Compensation and Benefits\n
