Key Responsibilities\n

• Conduct thorough psycho-social assessments to gather essential information about patients’ mental, emotional, and behavioral health.

• Develop, implement, and adjust individualized treatment plans in collaboration with patients, considering their evolving needs and circumstances.

• Diagnose mental health conditions and modify treatment methods as necessary to reflect changes in patient status and progress.

• Collaborate with a multi-disciplinary team including other healthcare professionals, family members, and community resources to develop comprehensive discharge plans that facilitate continuity of care.

• Advocate for patients and their families to ensure that they receive seamless coordination of care, appropriate referrals, and access to community resources.

• Maintain compliance with all relevant federal, state, and local regulations and adhere to established clinical standards.

• Monitor patient progress, making adjustments to treatment plans, while providing crisis counseling and supportive interventions as necessary.

• Stay informed about community mental health resources to refer patients to additional services, support groups, or specialized treatment programs.

Patient Population / Client Focus\n

You will primarily assist a diverse range of clients facing various mental health challenges including anxiety, depression, trauma, and relationship issues. Your ability to build a trusting rapport with clients from multiple backgrounds will be essential. You will be tasked with approaching each individual's mental health journey with compassion and understanding, which is critical for client engagement and successful therapeutic outcomes.

Qualifications\n

• Master's Degree in Social Work (MSW) or related field from an accredited institution.

• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Mental Health Counselor (LMHC).

• Thorough knowledge of community mental health resources and treatment modalities.

Compensation and Benefits\n

This position offers a competitive annual salary ranging from $90,000 to $100,000, commensurate with experience and qualifications. Additional benefits include:
