Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of individuals, families, adolescents, and groups, utilizing graduate-level clinical social work and counseling techniques to formulate care plans tailored to client needs.

• Initiate the treatment process by performing thorough initial assessments, triaging care, and designing individualized therapeutic strategies.
\n
• Engage in ongoing communication with the clinical team concerning client progress and treatment approaches, contributing to interdisciplinary meetings and documentation requirements to support integrated care.

• Exhibit a thorough understanding of the biopsychosocial model by addressing the physical, psychological, and social needs of patients across the lifespan.

• Deliver effective case management services for assigned patients, collaborating with community resources and healthcare professionals to ensure comprehensive care.

• Complete all necessary documentation ethically and in a timely manner, in accordance with clinic standards and regulations.

• Utilize evidence-based treatment modalities, including Cognitive Behavioral Therapy (CBT), to support clients and families in achieving their therapeutic goals as outlined in their individualized service plans.

Patient Population / Client Focus\n

Qualifications\n

• Must hold an active state licensure as either LMHC, LMFT, or LICSW.

Compensation and Benefits\n
