Key Responsibilities\n

• Conduct thorough assessments and evaluations of clients' mental health conditions utilizing telehealth technologies, ensuring accurate identification of their individual needs.

• Develop and implement tailored treatment plans utilizing evidence-based therapeutic methods, such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT), adapted to meet the unique situations of hospice patients.

• Work closely with case managers, physicians, and hospice staff to coordinate interdisciplinary care and provide holistic support that addresses both psychological and emotional aspects of patient wellbeing.

• Assist clients who are experiencing acute distress through crisis intervention services, demonstrating empathy, active listening, and clinical expertise in challenging situations.

• Participate in community outreach and education initiatives aimed at raising awareness about mental health, particularly in the context of hospice care, to reduce stigma and improve access to services.

Patient Population / Client Focus\n

You will be working with a diverse range of clients, including those facing terminal illnesses, their families, and caregivers. Qualifications\n

• A valid and independent license as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or a Doctor of Psychology (PsyD) is required.

• Possession of a master’s degree or doctoral degree in fields such as Social Work, Mental Health Counseling, Psychology, or Marriage and Family Therapy.

• Demonstrated cultural competence and sensitivity to navigate diverse client demographics, including ethnic, cultural, and socioeconomic factors.
