Key Responsibilities\n

As a Licensed Mental Health Counselor, you will be responsible for a range of clinical duties, including but not limited to:

• Utilize established social work theories and interventions, alongside FKC patient education programs, to perform comprehensive assessments of clients' psychological and emotional needs.
\n
• Support patients adjusting to life changes, such as dialysis, through tailored interventions aimed at improving psychosocial outcomes and overall quality of life.
\n
• Provide supportive counseling services, focusing on both immediate and long-term mental health goals, while adhering strictly to your clinical training and state licensing requirements.
\n
• Assist patients in navigating the transplant process with educational and goal-directed counseling tailored to their specific circumstances.
\n
• Facilitate referrals to community resources—including home health services and vocational rehabilitation—ensuring patients have access to comprehensive support systems.
\n
• Regularly evaluate and maintain knowledge of local vocational and educational rehabilitation programs to assist clients in achieving employment or educational goals.
\n
• Engage patients in discussions about their advance directives, educating both them and their families about these crucial decisions in collaboration with the healthcare team.

• Participate in regular interdisciplinary team meetings to assess the psychosocial status of patients, identifying strengths and needs that may influence treatment outcomes.

• Contribute to collaborative care planning, working closely with patients and healthcare providers to identify interventions that facilitate goal achievement and enhance the quality of life.

In this role, you will primarily serve a diverse patient population consisting of individuals facing various mental health challenges. Qualifications\n
