Key Responsibilities\n

• Conduct thorough, culturally sensitive bio-psychosocial assessments to evaluate patients' mental health, substance abuse, and medical needs effectively.

• Provide individual and group counseling services, focusing on crisis interventions, intake assessments, diagnosis, treatment planning, and ongoing therapeutic support.

• Document mental health evaluations, progress notes, and risk assessments with precision to adhere to regulatory requirements and improve patient care.

• Serve as a liaison between patients and various community resources, ensuring access to local support services including medication assistance programs.
\n
• Collaborate with multidisciplinary teams and Quality Improvement departments to develop processes for data collection and outcome measurement that enhance clinical practices and patient experiences.

Patient Population / Client Focus\n

Your primary focus will be on supporting patients in a hospice care setting, which involves working closely with terminally ill individuals and their families. This role demands a compassionate approach, as you will address not only the psychological dimensions of end-of-life care but also the practical needs of families facing loss. The ability to provide grief counseling and facilitate discussions around end-of-life planning is vital.

Qualifications\n

• Active and unrestricted LCSW license in the state of California is required.

• A Master’s degree in Social Work along with a minimum of three years of post-licensure clinical experience.

• Strong knowledge of therapeutic techniques, crisis intervention strategies, and sound judgment in clinical decision-making.

• Proficient skills in group facilitation and developing treatment plans to address complex patient needs.

Compensation and Benefits\n
