Key Responsibilities\n

• Engage actively in treatment-planning meetings, providing insights on patient progress, challenges, and adjustments necessary to treatment plans.

• Identify and effectively manage resistance and conflict among patients, families, and interdisciplinary team members by employing active listening techniques and conflict resolution strategies.

• Maintain comprehensive and accurate patient records, documenting assessments, treatment plans, interventions, and case closures per social work standards.

• Utilize the most recent DSM diagnostic criteria to evaluate disorders and guide treatment approaches, demonstrating an understanding of human growth and age-appropriate developmental tasks.

• Apply behavioral modification techniques, assertiveness training, and supportive therapy tailored to the unique needs of patients and families.
\n
• Collect and analyze relevant data about patients’ medical, social, and emotional circumstances through effective interviewing and collaboration with healthcare providers.

• Develop, implement, and routinely update individualized treatment plans based on ongoing assessments of psychosocial factors affecting patient and family well-being.

• Conduct therapeutic educational groups, documenting patient participation and feedback to evaluate effectiveness.

Patient Population / Client Focus\n

This role is primarily focused on providing care to terminally ill patients and their families. Qualifications\n
