Job Details Specialty: Home Health Discipline: RN Duration: Ongoing Hours: 40 hours per week (8‑hour shift) Employment Type: Staff (Full‑time) Responsibilities Home health registered nurse case managers lead and collaborate to provide management and delivery of patient care plans. You will support patients by delivering individualized home health visits, ensuring compliance with care plans, and coordinating with interdisciplinary teams.
