Key Responsibilities\n

• Conduct thorough client assessments to identify mental health needs and develop tailored treatment plans that reflect the unique experiences of individuals and families within the hospice context.

• Provide individual, group, and family counseling sessions, utilizing evidence-based therapeutic modalities to address issues such as grief, anxiety, depression, and adjustment disorders.

• Monitor client progress towards treatment goals through regular evaluations and adjust interventions as necessary to optimize care outcomes.

• Collaborate with an interdisciplinary team of healthcare professionals, including physicians, psychiatrists, and nurses, to coordinate treatment efforts that encompass both mental and physical health aspects.

• Deliver crisis intervention services to clients in acute distress, providing immediate support during high-emotion situations, including crisis calls or family conflicts related to end-of-life care.

• Maintain meticulous and timely documentation of all client interactions and treatment processes, ensuring compliance with ethical standards and legal requirements.

• Stay informed about the latest research, best practices, and therapeutic resources related to hospice care and clinical social work, integrating this knowledge into your practice.

Patient Population / Client Focus\n

Your work will primarily focus on supporting clients who are coping with terminal illnesses and their families. This includes addressing complex psychological needs related to grief, loss, and the emotional ramifications of impending death. You may work with clients from diverse backgrounds, requiring cultural sensitivity and an understanding of various coping mechanisms and belief systems.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• A valid and active state license as a Clinical Social Worker (LCSW).

Compensation and Benefits\n
