Key Responsibilities\n

• Conduct comprehensive assessments, develop personalized treatment plans, and ensure delivery of therapeutic services aligned with client goals.

• Support clients in their daily living activities, including meal planning, shopping, and skill-building to enhance their independence.

• Provide medication management assistance, including prompts, monitoring adherence, and managing refill schedules as directed by program guidelines.

• Facilitate transportation arrangements for client medical appointments and educate clients on utilizing public transport systems efficiently.

• Maintain an adequate supply of household essentials for clients, ensuring they have access to necessary resources to support their well-being.

• Forecast and manage client safety protocols, actively participating in ensuring safe practices within the counseling environment.

• Document and maintain accurate records of treatment progress, client interactions, and compliance with legal and contractual requirements.

• Act as an empathetic role model, encouraging clients to adopt safety measures and responsible behaviors in their daily routines.

• Communicate effectively with team members, providing updates on clients' progress and collaborating on treatment approaches.

Qualifications\n

• Possession of a valid and unrestricted LCSW, LMFT, or LPCC license in California is mandatory; associates-level licenses are not billable through Medicare.
