Key Responsibilities\n

• Conduct thorough and ongoing assessments that evaluate the individual needs of patients, utilizing data to inform treatment plans.

• Actively communicate with supervisors, direct care staff, family members, and other stakeholders to provide updates on the progress and adjustments needed to care plans.

• Create and present a tentative appointment schedule to supervisors, detailing home visits and the plan for progress monitoring.

• Coordinate and conduct home visits to ensure in-depth observations, comprehensive data analysis, and staff interviews as part of personalized care strategies.

• Implement program plans effectively, continuously assessing their success and adjusting strategies as necessary.

• Participate in interdisciplinary team meetings to discuss patient care, share insights, and collaborate on best practices.

• Prepare and complete monthly and quarterly progress notes to keep abreast of each patient's journey and therapeutic advancement.

• Provide additional training to staff to ensure adherence to the program plan and accurate documentation standards.

• Deliver follow-up interviews and additional home visits to reinforce support and address the evolving needs of clients.

• Maintain comprehensive and organized patient records to ensure accessibility for authorized parties.

• Serve as a liaison to the public and the broader mental health community to advocate for and support developmental goals.
