Bridge Way Reflections Therapy is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote position. Regular business hours are expected, but occasional evening and weekend shifts may arise based on the needs of the patients and your assigned caseload.

• Conduct comprehensive psychosocial assessments by interviewing patients, observing behavior, and reviewing medical histories to identify emotional, social, and environmental factors influencing recovery.
• Provide therapeutic counseling to patients and their families, focusing on managing adjustment issues, emotional distress, and coping mechanisms through tailored support strategies.
• Collaborate closely with the rehabilitation team to design and implement individualized treatment plans that address both clinical and psychosocial needs.
• Assist patients in navigating community resources, such as support groups, vocational rehabilitation services, and housing assistance programs, to foster smooth reintegration into their environments.
• Educate patients and families on available resources post-rehabilitation, ensuring they are well-informed about ongoing support opportunities.
• Play an essential role in discharge planning by facilitating transitions to home care or other suitable environments, focusing on sustained independence.

You will play a pivotal role in supporting patients through some of the most challenging times in their lives, promoting emotional well-being and social stability to maximize their chances of sustained recovery and integration back into the community.

• A minimum of a Master's degree in Social Work from an accredited institution.
• Possession of an active state license as a Licensed Clinical Social Worker (LCSW).
• Profound understanding of psychosocial issues affecting patients undergoing rehabilitation, along with established counseling techniques.
• Exceptional interpersonal and communication skills, enabling you to connect effectively with patients, families, and multidisciplinary teams.

• Experience working in a rehabilitation or medical setting, particularly focused on detox and addiction recovery, is highly desirable.
• A strong background in addressing adjustment disorders, grief, and coping strategies will enhance your effectiveness in this role.
